Actress Isabel “Izzy” Tate Passed Away On October 19
Isabel Tate, an up and coming actress who went by the name “Izzy,” unfortunately passed away on October 19 at the age of 23.
The actress’ talent agency, McCray Agency, shared the sad nwews on their Facebook page, writing that they were “deeply saddened and completely heartbroken” too announce her passing.
The agency shared that Izzy had booked the first-ever role she auditioned for on 9-1-1: Nashville‘s pilot episode, playing a woman in a wheelchair at a strip club.

9-1-1 Nashville is a spin-off of the popular 9-1-1 series and just premiered on ABC on October 9, 2025.
Kim McCray, Tate’s representative, revealed that the actress “had a rare form of Charcot-Marie-Tooth disease,” which was the cause of her passing. Charcot-Marie-Tooth disease is a nerve disease, which Alan Jackson is also battling.
9-1-1: Nashville Paid Tribute To Izzy During Their October 30th Episode
9-1-1: Nashville star Hunter McVey worked with Tate on the show, and spoke with PEOPLE following her death.
He told the outlet, “I was shocked to hear the news [about Isabelle’s death]. I didn’t know Isabelle prior to filming, but I couldn’t have asked for someone better to work with on one of my first ever days on set.”
He continued, “I got the pleasure of talking with her after the scene and she had such a great energy. You could tell she brought joy to a lot of people. I’m thankful I got a chance to work with Isabelle and that a part of her will live on forever in the pilot episode of 9-1-1 Nashville. Prayers for her family.”
After Episode 4 titled “Bad Case Of The Blues,” which aired on October 30th, the show paid tribute to Tate with a graphic before the credits rolled.
“In loving memory. Izzy Tate,” the text read.
